Avoid acquired/worsening decubitus (pressure) ulcers by actively treating lesions in “pressure” areas to prevent progression to decubitus ulcers.
- Accurately categorize ulcers to distinguish decubitus ulcers from other lesions.
- Implement wound care measures beyond topical dressings including offloading measures, support surfaces, measures, compression, etc.
- Avoid surveyor demerits in wound care such as F-tags (F684/F686).
- Diligently document to ensure proper procurement and billing for wound care Durable Medical Equipment (DME).
- Improve the wound care/skin condition component of the Minimum Date Set (MDS) for the nursing facility.
- Immediately address wound care related Situation-Background-Assessment-Recommendation (SBAR) to uphold the Joint Commission standards.
